After many years of working as a social worker in a hospital setting I recognized a big gap in service to our patients and their families. So many times we would send patients home with a million pieces of paperwork containing instructions, follow up appointments, new prescriptions, as well as names of agencies that would be following up with them.  I could sense the overwhelm for these families. And many times would have contact with these same patients once they were home and were still having questions.  

It is difficult to digest information when we are overwhelmed. I started Feet on the Ground to help patients and their loved ones navigate care whether they are leaving the hospital or just at home maintaining their independence. 

So, if you need help with “what to do next?” or “where do we go from here?”, Feet on the Ground can help with all things senior.

Do you live in a different city than your parent(s) or loved ones? I do! It’s hard to serve them when you can’t jump in your car and quickly help. I understand the worry this distance can bring.

-They aren’t answering the phone. Are they ok? Should I call a neighbor?

-They need to go to see a specialist and you can’t take off work to join your parent at the appointment.

-They have a specific item that needs to be purchased and you can’t get it to them fast enough.
